I'm trying to understand how take advantage of the fact that Dodge AC stacks when it comes to Mythal crafting.

Is it possible to put Dodge AC on anything but boots?

If not, how can one get more than +5 Dodge AC without it being part of their race or their class?

How can one craft up to the +20 Dodge AC limit onto gear?

Only for boots. You can't get more than +5 from gear on Amia. There are various other sources, however, such as Mage Armor, Haste, Divine Shield and Bard Song.
