Not really bugs, but ...
1, Maximus is not resistant to cold - which means he is being harmed by the "weather". I've noticed this recently when he died to a cold damage caused by the weather effect. 2, Fire giants in the very last location, with the royalty, are being affected by the area effect that reduces AB and damage if save vs. fire is not made. Even the King and Queen. Now it says this penalty occurs due to "unbearable heat", to which they should be immune. 3, Undead creatures around the Manor are occasionally fighting each other. Not sure what causes this, but once they spawn they are immediatelly after each other, which results in them ignore player and fighting amongst themselves instead. 4, The very same happened with the barbarians, although I have noticed they only killed the "bard" character that spawns with them (a woman barbarian with sling and bard songs). Once they spawned, each group always killed the bard right away and only after that they went after me. Amusing but confusing. 5, Of Gnomes and Golems quest, part II, where you have to find a "golem soul" - the desired golem does not appear for some reason. It took me a good two or three resets to finally spawn him - I do not know if that's intentional or not, but it really may be confusing to those who are not aware where he spawns.
We all know the Scourge quest is broken - could we maybe place the dialogue option that Uhm mayor and tar maker used to have to some NPCs in Wharftown? Because the sailor who takes you to the island with the Scourge is still in place, so all you need is an NPC who makes you a tar and gives you the second part of the quest. I think it's the easiest, fastest and most effective solution right now.

The golem has never been a 100% spawn. I spawned him a couple of weeks ago on my first try but sometimes it takes a number of visits before you see him. I think there's some luck involved (a die roll). What you're describing doesn't seem out of line, and I think it's intentional.
I've seen critters fighting each other too in some occasions, for example the wolves and the goblins in the first Caraigh zone. They never spawn together but sometimes you spawn one set without seeing them and you keep walking right into another spawn trigger and they both end up converging on you, then take to fighting each other. Must be a faction setting.

I mean - it's a quest related creature, I think he should spawn on regular basis as per the quest description. Otherwise it may be highly confusing to the player as why the creature did not spawn.

I don't think there was always a quest there, if my sieve-like memory is correct. So it's possible aquest was added to him later, and his spawn chance never changed.

Not sure if this is considered minor or not, but would it be possible to make dragonshape a little bit smaller? Maybe the size of EDK now or something?

Dragonshape is balanced around its size, so I couldn't give thta one a nod, personally.

Sometimes an ankle high rock placed too close to another ankle high rock blocks you in combat when everyone else just walks through. But having used the shape for weeks now the only thing that blocks me are doorways. And those doorways shouldn't exist anyway because shit bigger than/same size as dragon shape often lives in the room on the other side so who fuckin knows. I'm looking at you fire king.
