Very soon my mage will get to pick an epic spell, have no idea which one to pick: Hellball or Greater Ruin. If you could only have one, which would you chose and why?

would go for GR, myself. it's faster and you can't hurt yourself from it

Voted for GR. In PvE you can use EMD summon, or another PC to get easily through regular mobs. You can use GR to kill boss more quickly. In PvP you usually dont go against big crowds anyway and even if you do, you usually want to take down one specific target quickly (the one that is biggest threat to you - usually other mage).
Hellball seems risky, damage can be partially resisted (fire with fire reduction or immunities... negative with maybe protection from negative, leaving only 20d6 damage ... unlike 50d6 positive with GR if I am not mistaken). One good thing is that it knocks down enemies so with rogue by your side it can be a bit more interesting.

I don't see that anywhere in spell description, either NWNwiki or Amia spells sidebar changes. Is this for real? If so, for how long, 1 round? Thanks for the input so far. Exactly opposite what I expected. Thought Hellball was the better of the two.

I've never seen hellball knockdown anything on Amia. It does knockdown on some other servers, though.

Vanilla NWN Hellball does knockdown, but our version had it removed when it was updated and changed.
