I have read through the tiefling subrace information and I am wondering if I create such a character - if her traits were to have wings or a tail do I need to put in a special request to have this approved? If not, how do I go about applying such a visual to the character?

It's listed here that tails and wings need a request. You'll find the DC prices here aswell.

Yes, tails and wings require a request and the appropriate approval and DC payment for such.
